Our Care Practice
We adopt a multi-disciplinary approach, working in partnership with
Psychiatrists, Community Psychiatric Nurses and other health and social care
professionals to deliver high quality holistic care.
Each resident will have a keyworker who will be responsible for ensuring
consistency and continuity of care, and liasing with other agencies and
relatives to ensure that care and support meets resident’s current needs.
Each resident will be encouraged to be closely involved in the planning of their
support. Their Service User Plan will include individual care plans and risk
assessments to address identified needs and choices. We welcome the involvement
of family, friends and/or advocates whom the resident may wish to involve in
the process.
An emphasis will be placed on daily living, self care, interpersonal and
community living skills, without neglecting other needs which may be
psychological, cultural, spiritual, emotional or social.
We will promote community participation, social inclusion and the utilisation of
community services. Our practice is informed by the principle of unconditional positive regard.
